Here are the forms and instructions for the summer reading book reports.

1) Create a cover sheet on construction paper or plain copy paper.

    Include the---

BOOK TITLE

  AUTHOR

    ILLUSTRATOR

     A PICTURE OF YOUR FAVORITE  CHARACTER IN THE BOOK

       YOUR NAME

 THEN   Write a summary of the book on lined paper. You must have 3 neat and well written paragraphs of more than 4 sentences each. The 3 paragraphs should tell about the beginning, the middle, and the end of the book.  Then you may add your opinion of the book.

1. For the second book, first create a cover. Put your name and the title of the book. Fill in an area with a collage of words and/or symbols about the book. Include words that tell your opinion of the book. Make it colorful!

THEN  Copy and answer the form included here. Be sure to be neat, write answers in complete sentences using correct spelling and grammar. You may answer on the form. If you need more space, just use lined paper and put the number of the question you are answering before the answer.
